You file it yourself on the digital Libro de Reclamaciones, reclamos.servicios.gob.pe: choose the entity and office, paste what happened and what you ask for, and you can attach the PDF. If the platform is down, use the entity's paper book. The entity has up to 30 business days from the day after the complaint is registered to answer (article 14), filing is free (article 15) and they must give you a copy of the record. If they ask you to complete details, you have 2 business days (article 16.1). It is not for complaints about health services, which have their own rule (article 3.2), nor for challenging a written decision.
